The move is genuinely different from a substantive role, and it is worth understanding what changes on the pay side, on the pension side, and in the support you get on shift, before making the call. Why Healthcare Workers Look at Agency for Flexibility
People move to agency for real reasons. School-friendly hours that do not exist on a normal rota. A break from consecutive nights. Choosing the type of work they like, not just whatever the ward needs. Or earning more per hour by picking shifts that pay best. Most people want a mix of all of these.
Three patterns come up regularly:
- The parent with school-age children. Weekdays from late morning to mid-afternoon in a setting that works with the school finish time, plus one Saturday a fortnight when the other parent is home.
- The night-shift specialist. Five long nights in a row at the enhanced rate, then ten days off. Suits people whose sleep pattern handles it and whose home life accommodates the schedule.
- The worker keeping a part-time substantive role. Three days a week in their permanent post for the pension and predictability, two days on agency for the pay top-up and variety. Tax and pension treatment get more complicated, and worth taking proper advice on.
Agency is not a lifestyle upgrade. It is a different working structure where you set the boundaries instead of inheriting them.
How Shift Choice Works at Medsolve

Shift availability can vary significantly depending on client demand. While some requests come through on the same day where a client requires immediate support, many shifts are received several days in advance, and in some cases weeks ahead. At Medsolve, we aim to understand each candidate’s availability, preferred working patterns and longer-term goals. Through regular communication with both our candidates and clients, as well as ongoing feedback from placements, we are often able to identify opportunities for advance bookings, block bookings and recurring shifts that provide greater consistency.
Shift offers come through the Medsolve App, by phone, or by email depending on what works for you. Declining a shift is fine. Agencies expect it. The Trade-Offs Worth Budgeting For
Holiday pay is calculated differently.
At Medsolve, holiday pay is paid on a rolled-up basis and is included within each payslip rather than being accrued and paid separately when leave is taken. This means candidates receive their holiday entitlement as they earn it throughout their assignments.
NHS Pension does not accrue for agency workers.
Medsolve engages the majority of its agency workforce through an in-house PAYE payroll model. In some circumstances, self-employed arrangements may be available where appropriate and compliant with the nature of the work being undertaken.
Sick pay works differently too.
Statutory Sick Pay applies if you are off during an assignment, rather than the enhanced contractual cover a permanent NHS contract would give you. SSP only kicks in if you are sick while on a booking, not between assignments. If you have a chronic condition that means time off is more likely than not, this is worth pricing in.
If you are combining agency with a permanent role.
The Working Time Regulations cap your average weekly hours at 48 unless you opt out. Your substantive employer needs to know you are also doing agency work, and your tax code will need handling carefully if you are being paid PAYE in two places. Speak to a consultant about your specific situation before committing.
Questions to Ask Before You Move

- How does shift booking work, by app or by phone or by both?
- How much notice do I usually get for shifts?
- What happens if I accept a shift and then cannot work it?
- How is holiday pay calculated and when is it paid?
- What pension scheme will I be auto-enrolled into?
- What support is there if a shift goes badly or I have a concern?
An agency that answers all of those clearly is worth your time. Vague answers are the cue to keep looking.
